You may be requested to take and record your blood pressure at home by a GP or nurse.

  • You will be given a blood pressure monitor to take home with you from the surgery in order to do this.
  • You will be provided with a form (located below) which you will fill out over a 10 day period taking your blood pressure readings from the morning and evening.
  • Once you have completed this it can be handed into reception along with the monitor.
  • This will then be passed to the GP or nurse who requested it once averaged and they will get back in touch with you if any further action is needed.
