Sickness Certificates

You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC website.

Sickness CertificatesIf after the 7 days you are still not fit to return to work, you must contact the surgery and speak to the reception team. Your details will be taken and the reason for requesting a sickness certificate, this will then be passed onto the duty team for review, who will contact you to discuss if necessary.  

If requesting a certificate by PLEASE allow 48 hours for its completion.

Sickness certificates where possible will be emailed directly to you, please provide this to the receptionist.  Alternatively this can be posted out to your home address.


With your employer's support, the note will help you return to work sooner by providing more information about the effects of your illness or injury.
